Country-turned-pop-star Christmas angel Taylor Swift made a wonderful Christmas wish come true for one very special Swiftie yesterday.

On Saturday, while on her way home to Tennessee for the holidays, the "Wildest Dreams" singer made a surprise pre-holiday pit-stop in Grand Junction, Colorado to visit with 13-year-old superfan Delaney Clements, who was diagnosed with cancer in 2010. That same year, Clements began gaining a strong following on social media when hashtag #DelaneyMeetTaylor began trending on Twitter, which is precisely how Swift became aware of her fan. Grand Junction's Mix 104.3 FM, one of our affiliates, helped to make it all happen.

As reported by CBS Denver, after undergoing radiation treatment earlier in December, Clements was recently released home to hospice care. Yesterday, that home was brightened by a visit from Swift, who spent time with young Clements and her family and friends all afternoon, even taking a few silly selfies together, and giving the girl "the best Christmas present ever," according to Clements' mom Wendy.

"I was just laying down taking a nap in my room when my mom said I had a visitor," Clements gushed of the encounter with her "new best friend" on Instagram, where she posed in a photo with Swift while wearing matching red lipstick. "And I thought it might just be another person coming to see how I was. Then suddenly the most amazing miracle happens... The Taylor Swift walked into my bedroom and spent the afternoon with me just talking and hanging! I am beyond blessed for everyone's help and support! Thank you! Happy holidays!"

Something tells us that Delaney wont be shaking off this joyful visit anytime soon, and it's unlikely that Taylor will either.
